*To be eligible to earn the $50.00 referral fee, your referral must purchase one of the following: Live Teacher Virtual Written Course, Premium Access Video Course, Live Teacher Virtual Clinical Course (BC or Alberta), Face-to-Face Clinical Course (BC or Alberta), or Ontario Clinical Exam Course. You (the "Referrer") must submit this form prior to or no later than 30 days after the student signs up for a course.
